To participate please print forms by clicking on the links below, fill them out and send them to: 5818-B Seminary Rd., Falls Church, VA, 22041, with the corresponding fees. Checks should be made to Forever Dancing L.L.C. After the deadline, we will be still taking entries, with a penalty of $5 per dance entry. Be aware that late entries may not show in the competition program. Please view each form in your browser and print preview to ensure desired size before printing.
